Transition: Serious - Activities Unlimited


This section of the site is to help you if you’re in what’s known as transition – the period from about 13 to 25 when you move through your teenage years into adulthood.

Transition is about the new rights and choices you have during this time. if you have a Statement of Special Educational Needs (SEN), by law you are entitled to a transition plan, which is a document outlining what you want to achieve in your teenage and early adult years, and what support you will need to live as independently as you wish to. There are lots of people who can help you put your plan together and make sure that what you put in your plan happens.

If you do not have a Statement however you have a disability, and you think a transition plan would be useful, please speak to your school or Youth and Connexions advisor.
